The purpose is to facilitate the student&#8217;s move from school to post-school activities.<\/p>\n<p>Parents \u2013 Request that the IEP team begin formal transition planning for your child.\u00a0 Ask for a formal transition assessment <strong>(in many states between the ages of 14-16)<\/strong>.<\/p>\n<p>Work Readiness \u2013 Ask your school about pre-vocational services and school-wide jobs that might be available.
